That is why we offer a personalized approach to care and treatment. Our individualized method of care is respectful, educational and focuses on a person-centered journey. A complete approach to care.

To give patients the best opportunity for a positive outcome, all of the services listed above can be provided to patients at the same time they are receiving any CAPS service. Our services include care coordination. Coordinated care means assisting patients in connecting with other critical services such as transportation, childcare, housing, health insurance, legal services, food banks and social services, and more.

Care Coordinators are also available to help schedule dental, eye care, podiatry, nutrition and obstetrics/gynecology appointments. This combined approach to care ensures patients can receive continuing care after discharge. Regardless of outcome, patients can always return.
