Our Promise to You

We’re here to help our patients to reach their full health potential. We do that through a team approach to medical and dental care, with each patient at the center of their team. Our family doctors, specialists, nurses and medical assistants all work together to coordinate the best care plan for each patient. We also offer many additional healthcare services, like blood tests, X-rays, ultrasounds and a pharmacy on site to help make coordinating your healthcare convenient.

To become a new patient:

To have your child become a Syracuse Community Health patient through our school-based services:

To enroll your child to receive services at a school-based health center, contact your school’s office or school nurse.

Paying for your care.

We will never turn a patient away because they don’t have insurance or can’t afford to pay. That’s why we offer a Sliding Fee Program that allows us to discount your charges for medical and dental services based on your household income and family size. We will adjust only the amount that you must pay based on this sliding fee scale.

All patients are encouraged to apply for this program.
